Moving on to the location part, you'll offer an overview of the company's premises and their surroundings. Explain why this particular location is of interest, highlighting factors like catchment area, accessibility, and nearby amenities.

When describing the location of your private clinic, it may be beneficial to emphasize its potential to draw in customers from a variety of areas. Your competitive analysis should identify the indirect and direct competitors your business faces and then focus on the latter.

Everyday short-term processes include all of the tasks involved in running your mobile health clinic business, including answering calls, planning and providing mobile clinic services, billing insurance and/or corporate clients, etc. 

Long-term goals are the milestones you hope to achieve. These could include the dates when you expect to book your Xth corporate client, or when you hope to reach $X in revenue. It could also be when you expect to expand your mobile health clinic business to a new city.

Management Team

Balance Sheets

Balance sheets show your assets and liabilities. While balance sheets can include much information, try to simplify them to the key items you need to know about. For instance, if you spend $50,000 on building out your mobile health clinic business, this will not give you immediate profits. Rather it is an asset that will hopefully help you generate profits for years to come. Likewise, if a lender writes you a check for $50,000, you don’t need to pay it back immediately. Rather, that is a liability you will pay back over time.

Cash Flow Statement

Your cash flow statement will help determine how much money you need to start or grow your business, and ensure you never run out of money. What most entrepreneurs and business owners don’t realize is that you can turn a profit but run out of money and go bankrupt. 

When creating your Income Statement and Balance Sheets be sure to include several of the key costs needed in starting or growing a mobile health clinic business:

  • Payroll or salaries paid to staff

After the consultation, the doctor or the nurse will escort the patient to the front where all further arrangements will be made. A courtesy reminder card will be filled our for patients’ convenience.

The start-up requirements include purchase of a building for use by the clinic. The building was built in 1931 and has undergone a complete remodeling.  The total office space will be approximately 1,158 sq. feet and the lot size is 115.5′ x 66′.  There is also a storage shed which is 10 x 12 ft.  The office is located just off the main street and the office has blacktop parking for eight cars plus a generous carport, as well as a wide driveway entrance for patient convenience.

Management Team

Park Square Family Medicine will initially have two employees: a receptionist and a medical assistant. They will both be paid hourly wages and have health and dental benefits. As the practice grows, we will add additional personnel to help with referrals and additional responsibilities that will be needed at that time.

During the first year, Park Square Family Medicine’s physician (the clinic owner) will work full-time at the clinic, but will be paid directly by General Medicine, and all of his benefits and payroll taxes will be paid by them. There will be a mandatory staff meeting on Wednesdays at 12:00 noon. There will be a pre-written list of items that will address ways to improve the practice. Everyone is expected to attend. All employees, including the doctor, will be expected to continue to better themselves in regards to both skills and knowledge. The staff of the clinic will have specific duties that they will perform on a daily basis. They will be responsible for their individual duties and must complete them in a systematic time-efficient manner.

Once a month, Dr. Detroit will meet with each employee individually, and review their goals and performance for the month. Employees will have access to administrative consultants at General Medical for the first year, and will be expected to follow the suggestions that come out of joint meetings with Dr. Detroit and the consultants. Employees who consistently do a poor job will be written up and after two write-ups will be put on suspension. If the employee again fails to correct the problem that employee will be dismissed. All write-ups will occur in the presence of a witness and will be recorded.

Building A Website

With that in mind, building a website (or hiring a professional to build one for you) is the natural first step in promoting your practice online. Your website will act as the home base for all of your digital marketing efforts — meaning, every other digital marketing initiative you try should direct traffic back to your website. As the foundational piece of your marketing strategy, your website should be intuitive, attractive, and optimized for search engine visibility.

It will also be important to support your website with new, high-quality content once it is built out and launched. Starting an informative, active blog can be a fantastic way to highlight your expertise, reach engaged audiences, and support your positioning in search engine results.

Referrals & Directories

In most cases, the two most common means for finding new patients are to 1) utilize and nurture a referral network and 2) be on insurance contracts. This makes sense in practice; people trust other people to give honest referrals of a practitioner and their practice. They also want to ensure the provider is on their insurance network before moving forward.

With that in mind, you will need to develop referral sources to generate patient flow and to build the kind of practice that you want. Your referral sources will know your strengths and weaknesses and will refer accordingly.

In the online space, users often look to directories, insurance networks, and review sites to determine whether a provider is a right fit for them. Having active, complete profiles or listings on as many directories as applicable ensures you’re considered in the most relevant patient-provider searches possible. Additionally, directories and review sites act as an opportunity for current patients to leave reviews, providing invaluable feedback to support your practice’s long-term success.

A few examples of relevant, high-traffic directories you could sign up for include Google My Business , Psychology Today , GoodTherapy , or most social media platforms. Depending on the supply and demand characteristics in your area, however, being on insurance contracts and having profiles on insurance networks is sometimes all that is needed.

Revenue Revenue = Average fee per face to face hour x hours worked It’s easy to overestimate this number by overestimating collections rates, fees paid by insurance for a unit of service, show rate, and patient flow. We recommend budgeting for 20% less than whatever you come up with in your plan, at least until you get validation from real revenue.

Expenses Expenses = Facilities + Front Office + Back Office + Insurance

A basic rule of thumb for expenses is that they represent anywhere from 15-30% of total revenue for a solo practice. If you employ staff or outside services for front office and back-office work, they tend to be divided pretty equally between the three. If you do not employ staff, then these expenses are more heavily loaded on facilities. Make no mistake, you are paying for those front office and back-office functions with your time. This is where the concept of opportunity cost applies. The cost of your time is dependent upon either the value you place on your time or the revenue you could generate by seeing patients during that time. However, opportunity cost does not explicitly appear in your financial statements.

Fact Sheet 42 CFR Part 2 Final Rule 

Date: February 8, 2024

On February 8, 2024, the U.S. Department of Health & Human Services (HHS) through the Substance Abuse and Mental Health Services Administration (SAMHSA) and the Office for Civil Rights announced a final rule modifying the Confidentiality of Substance Use Disorder (SUD) Patient Records regulations at 42 CFR part 2 (“Part 2”). With this final rule, HHS is implementing the confidentiality provisions of section 3221 of the Coronavirus Aid, Relief, and Economic Security (CARES) Act (enacted March 27, 2020), which require the Department to align certain aspects of Part 2 with the Health Insurance Portability and Accountability Act of 1996 (HIPAA) Rules and the Health Information Technology for Economic and Clinical Health Act (HITECH).

The Part 2 statute (42 U.S.C. 290dd-2) protects “[r]ecords of the identity, diagnosis, prognosis, or treatment of any patient which are maintained in connection with the performance of any program or activity relating to substance use disorder education, prevention, training, treatment, rehabilitation, or research, which is conducted, regulated, or directly or indirectly assisted by any department or agency of the United States.” Confidentiality protections help address concerns that discrimination and fear of prosecution deter people from entering treatment for SUD.

The modifications in this final rule reflect the proposals published in the December 2, 2022, Notice of Proposed Rulemaking (NPRM) and public comments received from: substance use disorder and other advocacy groups; trade and professional associations; behavioral and other health providers; health information technology vendors and health information exchanges; state, local, tribal and territorial governments; health plans; academic institutions, including academic health centers; and unaffiliated or anonymous individuals. Following a 60-day comment period, HHS analyzed and carefully considered all comments submitted from the public on the NPRM and made appropriate modifications before finalizing.

Major Changes in the New Part 2 Rule

The final rule includes the following modifications to Part 2 that were proposed in the NPRM:

  • Allows a single consent for all future uses and disclosures for treatment, payment, and health care operations.
  • Allows HIPAA covered entities and business associates that receive records under this consent to redisclose the records in accordance with the HIPAA regulations. 1
  • Permits disclosure of records without patient consent to public health authorities, provided that the records disclosed are de-identified according to the standards established in the HIPAA Privacy Rule.
  • Restricts the use of records and testimony in civil, criminal, administrative, and legislative proceedings against patients, absent patient consent or a court order.
  • Penalties : Aligns Part 2 penalties with HIPAA by replacing criminal penalties currently in Part 2 with civil and criminal enforcement authorities that also apply to HIPAA violations. 2
  • Breach Notification : Applies the same requirements of the HIPAA Breach Notification Rule 3 to breaches of records under Part 2.
  • Patient Notice : Aligns Part 2 Patient Notice requirements with the requirements of the HIPAA Notice of Privacy Practices.
  • Safe Harbor : Creates a limit on civil or criminal liability for investigative agencies that act with reasonable diligence to determine whether a provider is subject to Part 2 before making a demand for records in the course of an investigation. The safe harbor requires investigative agencies to take certain steps in the event they discover they received Part 2 records without having first obtained the requisite court order.

Substantive Changes Made Since the NPRM

In addition to finalizing modifications to Part 2 that were proposed in the NPRM, the Final Rule includes further modifications informed by public comments, notably the following:

  • Safe Harbor: Clarifies and strengthens the reasonable diligence steps that investigative agencies must follow to be eligible for the safe harbor: before requesting records, an investigative agency must look for a provider in SAMHSA’s online treatment facility locator and check a provider’s Patient Notice or HIPAA Notice of Privacy Practices to determine whether the provider is subject to Part 2.
  • Segregation of Part 2 Data : Adds an express statement that segregating or segmenting Part 2 records is not required.
  • Complaints : Adds a right to file a complaint directly with the Secretary for an alleged violation of Part 2. Patients may also concurrently file a complaint with the Part 2 program.
  • SUD Counseling Notes : Creates a new definition for an SUD clinician’s notes analyzing the conversation in an SUD counseling session that the clinician voluntarily maintains separately from the rest of the patient’s SUD treatment and medical record and that require specific consent from an individual and cannot be used or disclosed based on a broad TPO consent. This is analogous to protections in HIPAA for psychotherapy notes. 4
  • Prohibits combining patient consent for the use and disclosure of records for civil, criminal, administrative, or legislative proceedings with patient consent for any other use or disclosure.
  • Requires a separate patient consent for the use and disclosure of SUD counseling notes.
  • Requires that each disclosure made with patient consent include a copy of the consent or a clear explanation of the scope of the consent.
  • Fundraising : Create a new right for patients to opt out of receiving fundraising communications.

What has not changed in Part 2?

As has always been the case under Part 2, patients’ SUD treatment records cannot be used to investigate or prosecute the patient without written patient consent or a court order.

Records obtained in an audit or evaluation of a Part 2 program cannot be used to investigate or prosecute patients, absent written consent of the patients or a court order that meets Part 2 requirements.

What comes next?

The final rule may be downloaded at https://www.federalregister.gov/public-inspection/2024-02544/confidentiality-of-substance-use-disorder-patient-records . HHS will support implementation and enforcement of this new rule, including through resources related to behavioral health developed by the SAMHSA-sponsored Center of Excellence for Protected Health Information . Persons subject to this regulation must comply with the applicable requirements of this final rule two years after the date of its publication in the Federal Register . The Department will conduct outreach and develop guidance on how to comply with the new requirements, such as filing breach reports when required.

OCR plans to finalize changes to the HIPAA Notice of Privacy Practices (NPP) to address uses and disclosures of protected health information that is also protected by Part 2 along with other changes to the NPP requirements, in an upcoming final rule modifying the HIPAA Privacy Rule.

HHS planning to implement in separate rulemaking the CARES Act antidiscrimination provisions that prohibit the use of patients’ Part 2 records against them.

Here's how to reduce your health-care costs during Medicare Advantage open enrollment

Certain retirees can now change their health coverage during Medicare Advantage open enrollment , which runs until March 31.

Medicare Advantage is health coverage provided through private companies that are paid by Medicare to cover your benefits.

If you're already on a Medicare Advantage plan , you can take advantage of the open enrollment period to switch to another Medicare Advantage plan or drop your Medicare Advantage plan and switch to original Medicare and perhaps also a separate Medicare drug plan. (Original Medicare includes Parts A and B for inpatient hospital stays and outpatient care.)

One of the biggest reasons Medicare beneficiaries may want to switch coverage is costs.

Recent research from the Employee Benefit Research Institute suggests that retirees may need to have substantial sums set aside to cover health care costs in retirement — up to $413,000 in one "extreme case" for a couple covered by Medigap with high prescription drug expenditures to cover 90% of their costs.

However, those enrolled in Medicare Advantage may need lower savings targets, the research found. For example, couples may need to have just $189,000 saved to have a 90% chance of covering their retirement health care costs.

There are potential drawbacks to Medicare Advantage, the research notes, particularly because those plans tend to have limited networks or require approval before covering certain medications or services.

While those six-figure estimates may sound intimidating, a better way to plan for health care costs in retirement is as an annual cash flow need, rather than lump sum savings, according to physician and certified financial planner Carolyn McClanahan, the founder of Life Planning Partners in Jacksonville, Florida.

"We need to look at this year to year instead of trying to predict 30 years of costs," said McClanahan, who is also a member of the CNBC FA Council .

Medicare open enrollment periods can be an opportunity to identify potential ways to save.

When a Medicare Advantage plan may be best

Medicare Advantage plans have largely been put in place because of baby boomers, who have become used to managed care plans in their working years, notes Darren Hotton, associate director of community health and benefits at the National Council on Aging.

The plans allow beneficiaries to carry just one card and usually come with co-pays. Medicare Advantage plans generally don't require beneficiaries to get a separate drug plan for prescriptions and may also provide for supplemental benefits that are not allowed under original Medicare.

"It's great for people who are healthy," Hotton said. "It's great for people who love co-pays and being told where to go."

Because Medicare Advantage plans come with an out-of-pocket maximum, they also are typically a fit for low-income beneficiaries who may struggle with high deductibles and coinsurance under original Medicare, he noted.

When to keep your options open   

Whether or not a Medicare Advantage plan is right for you depends on how healthy you are.

"If you know that you have a lot of medical services, it's probably best for you to just go to Medicare and a Medicare supplement," Hotton said. "But if you're healthy, you could save money, then this is probably the right option with Medicare Advantage."

Retirees may be tempted to go with Medicare Advantage once they see the lower premiums, particularly if they live in an area with a strong network, McClanahan said.

But the problem is those plans can be restrictive if you come down with a serious condition and need specialized care.

If you want to switch to Medigap — extra insurance provided by a private insurance company to supplement original Medicare costs — you would have to undergo specialized underwriting. If you're sick, you would be unlikely to pass and stuck with the more limited Medicare Advantage coverage, McClanahan said.

"If you're young and healthy, you don't know what disease you're going to get," McClanahan said. Consequently, she said she often recommends original Medicare coverage.

Hotton said he has seen beneficiaries who go back and forth between Advantage and original Medicare many times in their lifetime. The key is to be strategic. For example, someone who has a family history of health declines after 80 may elect to switch back to original Medicare when they approach that age milestone.

How to take advantage of open enrollment

If you're currently on a Medicare Advantage plan and are considering whether to switch during open enrollment, the first step is to visit your local State Health Insurance Assistance Program , or SHIP, Hotton said.

There, they can walk you through the Medicare plan finder, whether you can see how your current plan compares with others with regard to providers, prescriptions and pharmacies.

If there's a better plan, the help at your SHIP office may be able to help with that transition, Hotton said.

Alternatively, beneficiaries may also compare plans by visiting Medicare.gov .

Beneficiaries may also receive help by calling Medicare's 800 number . However, they may only send you a more limited list of the top three Medicare Advantage plans in your area, Hotton noted.

Because it can take some time to become fully enrolled in new coverage if you switch, it's best to get started now rather than wait until the March 31 deadline, Hotton said.
